While most issues between client and clinician can be addressed directly, some situations could require escalation. In compliance with the State of Texas requirements, we provide information below to assist your pursuit of a report, if you should find yourself in such a situation.
Request your Records
We are required to retain your records for 7 years after treatment, or 5 years after a former child client has turned 18. If needed, records can be requested via the client portal.

License Violation Concerns
If you believe a clinician has violated their code of ethics and/or scope of their licensing regulations, please contact the licensing board. This information was provided in your consent for services, including information of who to contact if you are a client seen virtually in Florida, Louisiana, or Virginia.
